The Ecology of Breast Cancer the Promise of Prevention and the Hope for Healing looks broadly and deeply into the origins of breast cancer and some of the factors that influence recurrence and progression after initial treatment. The book briefly discusses known risk factors for breast cancer, including family and personal history, genetic susceptibility, early puberty, late menopause, late age of first child or having no children, dense breast tissue, chest irradiation, current use of oral contraceptives, combination hormone therapy, cigarette smoking, and alcohol consumption. It goes on to explore what we know about other variables, their interactions, and the importance of taking a life course approach, since breast biology and later risk of breast cancer can be influenced by conditions experienced during fetal development, infancy, childhood, and adolescence as well as adulthood. The first section of the book proposes that a systems perspective or an ecological framework is best suited for studying the origins of breast cancer and designing interventions intended to prevent it and improve outcomes following treatment. A second section reviews numerous studies addressing the roles of nutrition, physical activity, environmental chemicals, vitamin D, electromagnetic fields, shift work, and stress. Examples of interactions among these variables show the value of a systems-based approach to research and interventions. Section three synthesizes this information and identifies practical opportunities for individuals, health care professionals, public health officials, community planners, businesses, schools, governments, and farmers to help reduce the burden of this disease. Biomedical scientists and clinicians have made enormous advances in treating breast cancer in recent decades. Improved outcomes are likely due to combinations of earlier diagnosis, better understanding of cellular pathology, and refined, targeted therapeutic interventions. For many people with the disease, adding weight control, dietary interventions, exercise, stress reduction, and social support to their initial therapy has not only improved the quality of their lives but also reduced the risk of recurrence. A public health perspective widens the lens to look at breast cancer patterns in populations and offers additional insights into prevention and treatment. An ecological framework accommodates both individual and public health points-of-view and adds new science. It expands the ways we can study and address this disease. Finally, applying the ecological framework more widely to other complex disorders, including other kinds of cancer, diabetes, asthma, cardiovascular disease, learning disabilities, cognitive decline, and dementia, is likely to improve our understanding of their origins and point to better strategies for prevention and treatment. Changes in diet and the food system, the built environment, social environment, and reductions in hazardous chemical exposures designed to address breast cancer will help reduce the burden of many of these as well.

With the publication of this fabulous book, we have finally arrived at a new and much-needed understanding of the epidemic of breast cancer. For decades it’s been all pink all the time pink ribbons/pink group parades/pink bumper sticker campaigns to ‘cure’ cancer. By the time we’re fighting against cancer, it’s really too late (perhaps not for the individual, but definitely for society).
The Ecology of Breast Cancer takes us to where we need to be – prevention. It is a treasury of virtually every significant study of breast cancer, each clearly described and analyzed. For ‘lay’ readers, this book becomes a road map of how best to prevent the disease. For scientists, it offers a unique trenchant summary of the most important scientific literature.
The author, Dr. Ted Schettler, is one of the nation’s leading public health scientists.
Breast cancer has reached epidemic proportions...because our nation (indeed, probably every nation on earth) has failed to address and redress its causes. Because of the way Dr. Schettler has collected and organized the studies, you understand the web of factors which enable the growth of breast cancer, from individual biology and lifestyle, to community and national conditions. “Breast cancer is an ecological disease as much as it is a disease of abnormal cellular growth. It arises from system conditions…the result of interacting multi-level variables that begin early and extend throughout life.”
In turn this analysis clearly discloses the places and ways in which to intervene for prevention, as individuals and communities and nationally. A particularly critical finding, study after study, is that most important time to reduce the risk of cancer is in the earliest stages of life, from inception to childhood to adolescence.
Chapter by chapter, the book reviews the influence of specific factors, starting with food. The studies discussed here range from the impact of consumption of meat, to soy, fruits and vegetables, even seaweed and mushrooms. “Diet in childhood and adolescence has a stronger link to breast cancer risk ...than diet in adulthood.” Another chapter examines the role of physical activity in prevention and recovery.
The chapter analyzing chemical and other environmental contaminants concludes that “early-life chemical exposures can increase breast cancer risk.” Dr Schettler explains “People are often exposed to chemicals that are mammary gland carcinogens in animal studies. These exposures can occur during fetal development, in the workplace, and in the everyday life of children and adults. With virtually no requirement for pre-market safety testing of most chemicals in commerce, it’s difficult to see this as anything but societal failure to protect the general public to exposure to hazardous chemicals almost certain to increase breast cancer risk.” (How often do you hear the ‘pink’ champions address this subject?)
The book concludes with a chapter that explicitly presents detailed guidance for individuals and families, and a national action plan to “look upstream at the beginning of casual cascades with multiple downstream impacts—the food system and individual diets, the built environmental and physical activity, the material economy and environmental chemical exposures,” giving us the base upon which “we can better design interventions,” from public health strategies to a reorientation of our nation’s food production.
